COVID-19 Site Questionnaire.

From Monday July 20th, ADISA is intending on summing its audit schedule. Initially this will be only full audits for new members and forensic audits for existing members until the end of August. From September 1st a normal audit schedule is to commence.

This form is to be completed by all ADISA members or those seeking certification to allow all parties to make a risk based decision on whether the site under evaluation can be attended by ADISA auditors with the safety of the site employees and ADISA auditing team being paramount for all.
